65 had total hysterectomy, 7 yrs ago on estrogen cream. Spotting one day, then stopped, what would cause this?

If you don't currently have a GYN then call the clinic that did the surgery and tell them what's going on and they will probably get you in to see someone soon. Doesn't seem to be an emergency (ER) or urgent (Urgent Care) unless you start bleeding again and/or there is pain or any other symptoms, then the triage nurse will give you guidance on what you should do.
