My mother lives by herself in small Kansas town with limited resources about 4 hours from me. For years she has had the assistance of a case worker (weekly or bi-weekly) in managing mental health, basic finances, and medicare/health insurance/prescription plans. I have power of attorney to assist and am not sure where to begin finding a new case worker and/or transportation assistance to help her get to appointments (she drives, but not on Interstates or highways). I can make the trip to her occasionally, but she benefits from having a local caregiver/assistant. Any suggestions?

The case manager that is retiring SHOULD be giving you resources to help you find another. They can not just leave their client base with no direction.
You can look up Geriatric Care Managers and they can do what you are describing.
But when I hear the term "Case Manager" I am thinking this is through some funded program. If that is the case then the Case manager has a supervisor that you could be able to talk to.
You could also try contacting your Area Agency on Aging.
In many areas Catholic Charities runs programs (this is not just a Catholic organization they help all)
Some Senior Centers have Social Workers that are aware of programs in the area.
