Excellent Home Health Care, Inc (EHHCI) was recognized and certified since December 2003 by Centers for Medicare & Medicaid Services (CMS) as one of home-health care agencies which is assessed to have high-quality home-health care services for promoting health and improving the quality of life.
Our patients are direct referrals from doctor’s clinics, hospitals, and skilled nursing facility. At EHHCI, we provide each of our patients the best quality of care to eliminate the re-admission process. Our staff understand, care, and comfort each of our patients for strong outcomes by working and communicating with Care Managers, Social Workers, Discharge Planners, and Physicians.
Our medical staff visits the patient within 24-48 hours after the referral from doctor’s clinic or patient discharge from the hospital or skilled nursing facility.
We accept patients with Medicare, Medicaid and Private Insurance.
We serve the counties of Cook, Lake, DuPage, McHenry, Kane, and Will.

Occupational Therapy
The Occupational Therapist assesses, evaluates and provides plan of care in the use of therapeutic, creative self-care activities of daily living and adaptation to their living environment
Speech & Language Pathologist
The Speech Therapist provides rehabilitation service for speech and language disorders including but not limited to Esophageal/Tracheal-Esophageal speech instructions.
Medical Social Work Services
The Medical Social Worker helps patients and their caregivers with the social and emotional difficulties that may result from illness. The Social Worker will know about community services such as meal delivery, finance, transportation and other services.
Home Health Aides
State Certified Nurse Assistants assists patients with personal care and hygiene, grooming and activities of daily living.
Medical Equipment Coordination
EHHC arranges for the rental or purchase of needed medical equipment including but not limited to special beds, wheelchairs and walkers.
