The service begins with an assessment of the Elder’s needs and the creation of a Plan of Care, which is a suggested plan of action. This can be particularly helpful when family members disagree on what the Elder needs or simply don’t know where to begin.
The assessment covers many areas of the Elder’s life, for example the condition and safety of the home, medication management, and psychosocial needs. The Care Manager can make referrals to community resources and local professionals.
