I, too, have cared for a family member with multiple medical problems and came to realize first-hand how important it is to have someone "at bat" for you while navigating the health maze of multiple doctors, hospitals, ever-changing medications, and time-consuming forms. You know your loved one is in need of more than you can physically provide. Yours and their desire is to move above just reacting to the most recent problem. It takes thinking ahead to avoid some pitfalls. With my nursing and personal experience, we can together find those ways that can help improve their everyday living experience, assist their health care providers in seeing "the overall picture", and give you some assurance that you're doing all you can.
