You are welcome. I would recommend you contact your local Area Agency on Aging (or sometimes known as the Office on Aging). They are the masters at aging issues and such.

It depends on who is wanting the assessment. If it is a hospital setting I'm guessing they are wanting to assess the patient for medical history, symptoms of current condition, their current lifestyle (i.e. do they drink excessively, are they exposed to children-potential children's illness risks, are they incontinent-could have a bladder infection). It really depends on who is wanting the assessment but it is basically to find out what services or needs can and should be met for the patient so that they receive the best care available to them.
