At the onset of dementia, what factors should we consider with my mother's finances? She's independent and on SS with a house and assets.

What happens if she is diagnosed with Alzheimers and doctors says she can't live by herself or drive? What financial precautions should we make sure is in place so she doesn't lose all her assets?
