My husband has a blister between the toes and the ball of his foot, it is red, painful and swollen. What could cause this?

Just for information you can check on websites such as WebMD, eMedicine, etc.
Foot issues can become serious very fast. It could be a simple friction blister or an ulcer. Does he have Diabetes?
He really should see his doctor soon. Good luck
