How do I move my mother to an assisted living facility closer to relatives?

I had the same problem. My mother has dementia/alzheimers and lived 400 miles away. She was having outburst and I had her hospitalized for a few weeks. Then didnt know what to do. Had no help where she lived and no help from hospital. Thank God a friend told me about ambulance service that will transport people that cant be driven by family members. Only downfall is they strap them down for the ride. But Mom got here safe and was so happy with the new assisted Living.
They also have churches that will help. But a family member has to ride with them. Same with flying. Im not sure of your parents condition but these are the only options available when I checked. Hope this helped and good luck
