I am looking for legal information, the nursing home and hospital violated health wishes. How do we proceed?

There was a treatment given to my father while in the hospital that was not agreed upon and we as a family were informed after the fact. Also the nursing care facility did not care for him well and he ended up near death in the ER. looking to see how to take action legally against both???
