I need a template for a physician's letter to give to Medicaid for a caregiver waiver on child ownership of parent's home.

You might want to check with Medicaid or your physician to see if they have a letter for a waiver. I'm not quite sure what you are asking here but both of them should be able to help you. If not check you local Agency for Aging.
