Providing your sister is willing to give the POA over to you, then do it. I took my mother-in-law to her bank and filled out the paper work there. Not sure about the medical part tho, that's probably done via a lawyer.
Please give a little more information and people may be able to help more. Is your sister opposed to hiring caregivers, or has she just ignoring your mother? If your sister is ignoring her duties as POA and your mother is competent, she can also appoint you as her POA. There can be more than one. If she is not competent, you can talk to her lawyer about getting a court-appointed POA.
