Can anyone recommend an excellent acute rehab for my dad in the metro Detroit, MI area?

My father is an ostomy patient with a very difficult case due to heart issues. He was at Rivergate in Riverview but only lasted 3 days twice and ended back up in the hospital due to dehydration. We would like to see if there are any highly rated subacute rehabs that are excellent in not only rehab but monitoring and intervening before he gets to a situation where he ends up back in the hospital.
