Can I update new phone numbers, addresses and name change on my mothers POA, Living Will, and Medical Proxy by myself ?
My mothers POA, Living Will, and Medical Proxy was done in 2007. I need to update with new phone numbers, name change of care giver. Can I do this by myself or do I need to get a lawyer? My mother does not have anything to her name.
Do you have the name of the attorney who did these originally? You might call and ask him to add a paper attachement providing the updated info. Make sure you ask if there willl be a charge for this.
Is your mom able to go with you to a bank or other notary? You could write up the new information as an attachment and with your mom (she would need to be competent) with you have the document witnessed by the notary.
Whichever way you go, make sure the attorney, drs offices, banks, insurance, etc. have a copy of the updated contact information.
