My mom is in a rehab facility but she wants to go home. That would be to the home she shares with my 92 yr old dad. What options would be available to her for in home care so my mom and dad (they have been married 75 yrs) can stay in their home together. Assisted living isn't an option for them because my dad refuses to go anywhere but their home.

Check in often to make sure it is working out for them.
You might also enlist a trusted friend or neighbor or their church to check in from time to time if you are long distance. Another option is sometimes call their drs office and nurses sometimes freelance in evenings or weekends and will provide skilled nursing care. They might ask around the office and be able to at least give referral or pass along to one of their nursing friends who will do this.
Meet with rehab team soon to find out prognosis and their recommendations based on your parents situation. They don't always get the full story...and parent may be fibbing about their home situation, that they have daughter nearby...who will be taking care of them, etc. my mom did this and they were releasing her to a situation where they thought she was provided for and then we were left scrambling to put alot of care management in place...I live 6hrs away with full time job. I wasn't going to be around to look after mom yet that is what she told care team.
