Here is a link from your home town's website (from your profile) Faith in Action / Drive a Senior - Senior citizens may apply for complimentary rides from community volunteers to the doctor, grocery store and appointments. http://driveasenior.org/
Try also taking your dad to adult daycare. Contact your dad's medicaid or VA or other insurances he has if they cover adult daycare. I take my grandpa who also has dementia to adult daycare and his medicaid pays for it all. Adult daycare takes off a HUGE burden off of the caregiver's hands so the caregiver can get respite for themselves or when they need to work.
If your father is a veteran, then transportation can be provided. If he is a senior, does not drive, then the area disabled transportation system can pick him up. Call the senior social services agency for your state.
