Any suggestions for coordinating care for a parent who wants to stay in their home?

What have you found works well to keep track of and coordinate all of the services and people coming in and out of the house?

Keep a log of dates, times, who was there and what they did. A single coordinating agency like the VNA (Visiting Nurses Association) will provide oversight of PT, OT, Medications and informing the MD, but you still need to keep a log and ask for hard copy of any test results including bloodwork. I make a point of staying in touch with the RN assigned to mom. When the RN calls the MD things happen a lot quicker than if I call them. If mom has chest pain or dizziness, write it down, because mom then "forgets" to tell the nurse about it. I have the nurse's cell phone number and she has mine. We have a good laugh when mom tries to sneak something by us. This morning mom went out to try and shovel the sidewalk. BUSTED! The nurse gave me a heads up, so when mom calls me to say she feels rotten, I already know why.
