How can I move my Mom from a nursing home in Sunnyvale (SFO) to Encino?

My Mom is a resident in the White blossom Nursing home. We don't have many family or friends in that area to go and visit her, therefore we have decided to move her to Los angeles(Encino) where there are more friends and family to go and visit her. I would really appreciate if you could show me the best and quickest way to do this.

Ask the present nursing home to coordinate the transfer. They can arrange sedation and paramedics will have the proper tools to deal with any emergency enroute. Get as many personal items to the new place as possible so there is little noticeable difference in the room. She may need anxiolytics for a week to settle in.

I believe you can hire a medical transport to bring her down safely. I don't know what the cost would be however. That's a long trip.
