I'm moving my grandpa from Nevada to Kansas. How hard is it to move healthcare to KS?

My grandfather has finally agreed to move from NV to KS to be closer to family. He currently has Senior Dimensions as a medicare HMO. Does anyone know what steps I need to take to move his healthcare? Any advice would be greatly appreciated.
