My grandmother lived overseas, and she had a bank account here. Sadly, she passed away last year. She left a bank account in CA with less than $2000. My mom is having trouble getting the form so she can have the account closed and cashed out. And frankly, so am I . I can't find the form anywhere (Court site, the bank said they don't have one but the Court site says its at the bank). Anyhow, my parents could really use the money and my mom is allowed to have it. How do I get an Affadavit form for her?

In Illinois, what you'd need is called a Small Estate Affidavit. Don't know what it's called in California.
Just "affidavit" tells you nothing. There is more to the name than that. California probably has dozens and dozens of different types of affidavits.
