Elderly aunt living in Binghamton, NY. Who to contact for help?

My aunt who is 89 lives alone in Binghamton. My mom who is 77 is her power of attorney. My mother is not in good health herself. Wondering who we could contact about help for the aunt. She is suffering from pulled muscles in her back and unable to get around, make meals, clean her house, etc. I feel she is getting forgetful and not taking her meds when she should or eating like she should. We live in Montrose PA and unable to get up to Binghamton much. Please give information on who to contact or what programs are available in Broome county. Thank you!
