How did you "apply for Medicare"? Something here is wrong. Government agencies send official letters on official letterhead with a number to call if there are questions. Just call Social Security 1-800-772-1213 or Medicare 1-800-Medicare to get some answers. This is fishy.
Also, Medicare is a Federal program that is based on a person have worked for a certain number of years and having "paid into" the system, both via their own contributions and those of their employer. Your mom might also be eligible for Medicaid, a state run system which low income folks are eligible for.
IK - is English your second language? If so, see if you can get the letters you have received translated. MediCARE since it is a national & federal program has to have their correspondence available in other languages. But these are usually the ones with large population groups (like Spanish, Vietnamese,). See if your local church or a local social service group can help you with all this.
If you go to the Medicare.gov website, you can find a phone number to call, also information about how to file an appeal. Did this letter come from Medicare itself, or from a Healthcare provider like a doctor or hospital?
Did your Grandmother receive such a letter? I can't understand why you would want to see a sample. Could you explain a little more what the problem is, so that we can help?
