Mid 60's Mom, health issues, living alone 10 yrs, won't allow visits to home & blocks inquires into health & money. Any ideas?

We have tried to discuss her future but she diverts all attempts. We are often blindsided with financial emergencies, and use these events to discuss her budget. No change. We know she has health issues but she won't discuss anything in a meaningful way. Her isolation is a clear path to disaster and we don't know how to penetrate all the barriers she uses to block our efforts to help her. We know that she cannot manage money, and is probably being scammed by "friends". Recent changes in health prevent her from working part time to supplement her income. We suspect that she is a hoarder but can't prove it because she won't let us visit her home. Are there any support groups or other resources for this type of situation?
