How do I get my Mom with dementia and psychosis to a facility to get her cataract removed?

It's to the point she doesn't see very well. She only wants to go to the doctor that removed her other one but that is not possible. I moved her from IL to Arizona to be closer to me. She doesn't actually remember the drive down here so she thinks she is still in Illinois.

She is very smart in some ways. She thinks she still lives in IL and I just moved her. Once I get her out of the Dementia care facility she will know she is not in Illinois. Plus don't want her to have a fit after I've made the appointment and get her there.
