Who to call about w/c van transportation?

Are there services in towns that offer w/c van transportation needs? Who do you call to find out? Also, who would you call to find places that let handicap people volunteer so they aren't just sitting around all day? Again, transportation would be needed. Something like this may not exist but thought I would ask. Mom really hates being home bound with nothing to do. She can't walk at all so is w/c bound.
