What is the exact form I need to help my Mom apply for Medicaid?

She is 68, real frail. Cannot live on her own anymore. Needs intensive assisted living or simply nursing home. She has no income except SS. She was denied once for Medicaid here in VA but that is because she was living in Florida. I applied again after she was her in VA for about 6 months, but never heard back. So again, where do I get the form; how do I do this. Please help. Thanks.
