What happen when the elderly person owns a broken down home but has no money?
You can't get medicare and can't afford to pay for assistance.

Maybe you mean Medicaid? What sort of assistance? Are there medical conditions?

Does this person get SS? If so, the nominal Medicare premium is deducted from SS. Considering what other health insurance can cost, Medicare is cheap.
Why isn't this person able to get Medicare? Could you elaborate please?
