Make an appointment at your local social security office. Bring what you have. Hopefully, you'll get an employee that actually wants to do their job and take the time to help you. If they can't resolve it right then and there, they should be able to tell you exactly what you need.
You're probably not contacting firms with the practice groups that include SS. These firms probably would also handle filing for disability and workers' comp claims.
I googed the NY State Bar Assn. to find some of the firms for you but got a message the that function of the site is still in progress. You could follow this instruction and contact the Webmaster, or better yet, call the BAR Center at (518) 463-3200 and request web assistance.
Here's a list of attorneys in Sodus, Wayne County, who do worker's comp issues; contact them and ask if they handle SS issues as well.
I have my employer W-2's, but I'm advised that will not be enough proof. What else is acceptable? My initial benefit check is due to arrive mid-month. I've gotten no help at all, just frustration and the 'runaround' from SSA. I've called a dozen lawyers, but nobody handles SSA related cases or problems. Not even my local Office for the Aging is any help, deferring me back to the state bar association for another referral (13). Before hanging up, the woman said, "Good luck." I answered seriously, "I'll need it." She laughed out loud! Not what I needed to hear from a government organization who are there to assist untangling sticky problems...

Better yet, these attorneys handle SS disability work. They might also handle the kind of case you have.
