When to get hospice involved?

Mom has really bad dementia and is in stage 4 renal failure. She will not be having dialysis because she also has a bleeding condition. We may be having to move her to a mattress on the floor because she wants to out of bed anywhere from 4-10 times at night. Is all of this a situation one in which we ask for hospice? I think that part of her confusion is due to the urea building up in her blood stream. She has been going down hill since late Nov. She is a huge fall risk and very unstable when walking so we use a wheel chair to move her around as much as possible.
