What form do I use for a doctors evaluation for cognitive impairment, dementia, or Alzheimer's?
We need to have an evaluation done on my mother (WW2 veteran's benefits) for cognitive function so I can take over her VA benefits management. She needs to have greater care. What form do I give the doctor for him to fill out?
You need the VA form 21-2680 dated May of 2015. They are accessible on line and I strongly recommend you fill it out with all the details and then have the MD sign it. NEVER rely on the doctor's office help to do this, they will make errors and omissions that delay or deny the benefits.
