Do I need a form to talk to my mother's doctor and also to let a friend talk to her doctor?
My mother wants me to talk to her doctor. And a close friend to talk to her doctor. And a caregiver to talk to her doctor. We had a bad experience with an unscrupulous niece talking to the doctor and we want to eliminate her specifically.
The forms are HIPAA [The Health Insurance Portability and Accountability Act]. Doctor offices have these forms, and you can also find them on the Internet.
The next time you go with your Mom to a doctor's appointment, ask to update the form. It would be best if your Mom said directly that she wants you to be able to obtain information from her doctor. Same with your friend. Same with the caregiver.
