I need help with long term care covered by Medicaid and Medicare.

My mom is a 20 year cancer survivor. She has breast and brain cancer when I was a child and was a normal healthy functioning woman until about 8 years ago when she had a stroke. She has been gradually going down hill the past few years, but about a month ago she all of a sudden would not walk and her mind is not there. We took her to the hospital where she stayed 8 days and they sent her to a skilled nursing facility for 20 days, but we were told that her Medicaid would only cover the 20 days. She came home yesterday and is still currently unable to walk and her mind is not there. We have 18 stairs when you first walk into our apartment so having her at home with us is not safe for her. She needs more help and I am able to give her. I need advice on finding out how Medicare will cover her to live in a long term care facility because I am unable to afford anywhere for her.

Adorkableash, usually after Rehab [skilled nursing facility] the Case Manager would have Occupational Therapy come out to assess where the patient will be living. Sounds like that wasn't done. Otherwise the OT would have immediately seen the stairs and know that your Mom wouldn't been able to come back home.
Contact the Case Manager and tell her what is happening.

Medicaid will fund her nursing home care but you might not be able to pick her facility. Many nursing homes have only so many "long term Medicaid beds." This means after so many days, the patient will have to move. The facility won't want to keep the patient any longer. Some nursing homes will take a patient on Medicaid right off the bat but many won't. Let her doctor know that you can no longer provide care for her at home and she needs skilled care. They can help you take it from there.
