Where do I find the applications to apply for home care and aid and attendance?

I am taking care of both my parents and would like to apply for the programs to help out with cost. Where do I apply for these programs? I can't work full time anywhere because of the continual appointments and in home care they need. Where do I find the applications to apply for Home Care and the Aid and Attendance programs? My Father a a war veteran.
