Lori, you would need to have Mom apply for Medicaid. Once accepted, one would need to follow the rules and regulations. Since your Mom signed her house over to you with the 5 year financial look back, it could be possible that your Mom could not use Medicaid for awhile. She would have to self pay for the nursing home equal to the equity of the house.
Now there are exceptions, as Glad mentioned above, if you have provided 24/7 care for your Mom, with proof for the past 2 or 3 years, Medicaid could overlook the name change on the deed.
I also recommend using the services of an Elder Law Attorney, as they are familiar with the State laws for Medicaid the the maze one has to go through.
You need to see an elder law attorney. Since you have provided care, that was medically necessary as determined by her doctor, transfer of the house may be exempt under Medicaid recovery.
