Section Q is a fun new form from Medicare/Medicaid. The "new Q" asks s resident "Do you want to talk to someone about the possibility of leaving this facility and returning to live and receive services in the community? If the resident answers "yes" a referral will be made to a third-party, independent community organization (local contact agency) to provide them information and potential assistance in relocating. You have to put their answer, as stupid as it sounds, unless they are adjudicated incompetent.
People unable to care for themselves don't get all their "wants" met. Sometimes, their choices are limited, and going home is not an option, at all.
Maybe some of their needs can be met, but not at home, because once home, many who are used to getting all their wants met will not allow help in after awhile, and end up re-hospitalized.

And once again, the forms method fails to get the correct assessment and needs of the patient met.
