Have a document in my hands from nursing home that they falsified. Now what?

I have asked questions about this ongoing issue on this site a few times. My mom, 69 with MS, was incorrectly prescribed a too high dose of Baclofen which caused to her to be lethargic and confused and then she went through withdrawal of the drug which caused her extreme pain and mental changes where she basically temporarily went crazy (not at all like her, she is very mentally competent). Now she is bedridden and we are unsure if she will survive this. Well I just filed a report with the state on Friday. I now just noticed that on a form from the nursing home, dated about two weeks ago, discussing her change in pain and how she needed more drugs. It states on this form that I was notified. I was not. That day I did not visit my mom because my son had pink eye and we were at the Drs right when it was claimed I was called and we avoided the nursing home because of his pink eye. I also checked my verizon cell phone log and they did not call me. What is the best step in action for all of this going on? I will call my contact for the state for my complaint against the facility to add this falsified report. Should I contact an attorney also? We are not looking for money against a case, I want the correct actions taken against this nursing home and the faculty responsible.
