When it comes to Medicaid, it is run by your State. Funds are gathered from the State taxpayers with some Federal Funding. Thus, each State is different when it comes to programs.
As for paying a relative to be a caregiver. If it is coming out of your Mom's pocket [your profile shows your Mom has memory issues] it would be whatever is agreed upon. If Mom needs full-time around the clock care, you would need three separate caregivers, each working an 8 hour shift.
Now with Medicaid, your Mom could go into a nursing home and Medicaid will pay for her care. You would need to call Medicaid to see what all is involved in your State, and if any reimbursement would be needed.
