I am on SSD. How do I plan for my elder years?

I was a victim of a horrible kidnapping and 10 years later, I was gang raped on a 3rd date. I was working and in college at the time of the 2nd one. I ended up becoming a recluse, afraid to leave the house. I am on SSD and get Medicare and Medicaid. I worry about what will happen when I reach a point that I no longer can take care of myself. Will I end up in a state funded nursing home? I do not know how to prepare for the future when I barley make enough to eat. I don't have a husband and I have no children.
