How do you write a guardianship letter?

The judge wants me to inform my siblings in writing and have them sign the document stating that I have requested to take gaurdianship over my mother. I have no idea what to write on that letter. Also, one sibling can not be reached. I have no idea where he lives or works and I do not have any communication with him.
