Assisted Living Issues, ie; what has happened, what have you seen, what has your loved one or you experienced.

I am starting this post to hear of genuinly bad experiences and true problems and events that you and your loved one have experienced in Asisted Living or Nursing Home facilities. And to date, I would like to know if you've taken action, if so, what action has been taken and what have the results been  (if any.)
