Is the SNF physician to be listed as his primary care doctor?

Spouse in snf receives care from the snf doctor. He has only seen outside providers - ER hospital, cardiologist, and a pain management doctor. He is being referred to a dermatologist, so I need to list a primary care doctor. Is the snf physician to be listed as his primary? He no longer sees the previous primary for anything.
