Is medical history still relevant once someone is in a nursing home on palliative care?

Mom just moved into a new NH and I have doubts that any of her medical history has followed her. Since I believe in a holistic approach to medicine my first instinct is to make sure that her history of Atrial Flutter, TIAs and brain scan that "lit up like a christmas tree", her sudden inexplicable decline 5 years ago and the decision to discontinue almost all of her BP/anti platelet/cholesterol lowering meds are known and considered in her future care plan. BUT, given her age (97) and that we have been treating her palliatively for several years already is it even relevant anymore?
