How may family members can be added on a HIPAA form?
If care is already in place for an elderly person, from any of the following types of providers: a care home, a hospice company, and a physician; and if one family member is already named on the HIPAA form, how may other family members be added?
Keep it simple, as the care home and physicians are busy and wouldn't have time to check to see who's name is on the HIPAA list if there is a long list of names. Have one or two names that the patient requests, actually it is up to the patient to place the names.
As for Hospice, Hospice prefers to have only one contact person to call to give updates, and it will be for that contact person to call the rest of the family.
