How do I obtain my mother's "bank statements" from her Social Security deposits for purposes of applying for Medicaid NH coverage?

My mother suffered a devastating stroke. After a period of time in a special ICU, she was transferred to a NH/Rehab facility. Her Medicare coverage is expiring. The facility requires 3 months of "bank statements" from her SS direct deposits to her debit card in order to apply for the Medicaid coverage. Both the finance office in the NH and I have been unsuccessful in acquiring this information. Please, advise me on the most efficient way to obtain this data as fast as possible.
