My Dad's Adult Day Center doesn't have email, and they keep sending forms home with Dad for me to complete. I complete them and put them in the bag with his bath supplies with hopes that they will end up in the hands of the right person -- no such luck. We're not sure what's happening to the forms, and Dad can't remember. I guess I could mail them back, but I was hoping to avoid yet another item on my must do list. I don't even have stamps. Maybe visiting the Post Office is best?

I just asked the director to send the form again and unfortunately, I'll just have to go to the post office and mail it. He was willing to fax it, but my office doesn't have a fax machine. I searched for a service online that would allow someone to fax something from a traditional machine to my email, but no such luck
Oh well...

If you are taking your dad to the adult day care center, couldn't you just go in with him and give the completed forms to the correct person?
