Suggestions for keeping up with a form?

They have a fax machine but they don't have email? Sounds like they are stuck in the 90's, almost nobody still uses fax. Hope these forms aren't time sensitive.

I don't take dad to the center, so I think that's where the breakdown is. I WISH they had email, but they don't do the technology thing.

I just asked the director to send the form again and unfortunately, I'll just have to go to the post office and mail it. He was willing to fax it, but my office doesn't have a fax machine. I searched for a service online that would allow someone to fax something from a traditional machine to my email, but no such luck

If you are taking your dad to the adult day care center, couldn't you just go in with him and give the completed forms to the correct person?

Ask at the daycare if they really need the form, or if  sending an email granting permission would suffice. And make sure you are talking to someone with authority, sometimes those on the front lines can't think outside the box.
