My mom has Celiac disease, COPD and has trouble swallowing and I am having trouble finding even a group home for her diet. Any suggestions?

Her Gluten-free diet also has to meet Level 2 Dysphagia criteria (soft food). She is also incontinent and walks so slowly with her walker that she cannot get to the bathroom in time. She is on oxygen at all times. She and my dad currently live in their own home in a retirement community but he cannot take care of her. He might go with her to a group home but he would rather stay in their own home and go visit her.
