Can I have my family member removed from one facility to another?

Why do you want to move this family member out of their current facility? Is the facility a nursing home, assisted living, or Independent Living? What are the medical issues? Does this person have memory loss? Is the person using Medicaid to help pay for facility or is the person self-pay?
