What to look for in a care facility when placing someone with dementia who needs total care?

Friend's husband had a massive stroke 11 years ago. Wife has been taking care of him for 11 years (no outside help) but his condition and mental state has deteriorated to the point she needs to place him. What suggestions do you have for her to choose the best place. He is total care w/ dementia. He is 86 and she is 80. He has MRSA, C. diff, diabetic as well.
