If you go to the court, there will almost certainly be counter staff who can help you with the forms. You will need to take a clear brief statement of the claim, including dates. Take with you any written evidence you have, and about $100 which should be more than enough for any filing fees.

If your claim is legal and under a certain amount of money (depends on jurisdiction) you can file in small claims court. No lawyer needed. The cap can be 3k to 5k. I would think u need to go to your county courthouse to do this. There probably be a filing charge.
