My mom is taking care of my grandmother, who we feel has all the signs of dementia. What is the process for getting her into a facility?

She can barely get around. She walks very slow and she is barely remembering things. The thing is that she doesn’t want to be in a home but she really needs to be in one. It’s taking a great toll on my mother. How would my mother go about getting my grandma into a nursing facility? What is the process?
